MGIMS PMT Submission of Application Form
- The application form complete in all respects can be submitted personally, at the Admission Cell of the Dean’s office or can be sent by registered / speed post.
- The application form (all four pages) should be accompanied with following documents:
- A demand draft worth Rs. 1500/- (Rs.1600/- if sent late) for examination fee drawn in favor of Kasturba Health Society payable at Sevagram/Wardha
- Four recent passport size photographs taken on or after 1/10/2012.
- The window envelope provided with Prospectus from MGIMS with postal stamp of Rs. 25 affixed on it.
- One acknowledgement card affixed with Rs 6/- stamp, for the receipt of application form.
- One attendance card with attested photograph and signature of the candidate in the box below the photograph only.
- Attested Xerox copies of rural claim certificates (para 3.2), if candidate has applied under rural category.
- No originals or attested copies of qualifying examination or Category claim or eligibility certificates should be attached with the application form. However, all these have to be produced in original at the time of verification/ admission following declaration of results. These certificates/ documents must be kept ready, as no additional time, what so ever for any reason will be given for preparing, production or the correction of any documents once called for verification / admission.
- The completed application form should reach the Office of the Dean, Mahatma Gandhi Institute of Medical Sciences, Sevagram 442102 with a demand draft of Rs.1500/- as examination fee.
- Application form will be accepted till 5th Macrh, 2013
- Forms submitted without the examination fee will be rejected and no admit card will be sent.
- Incomplete application form will be rejected.
- The cost of prospectus, application form and examination fee shall not be refunded.
